Diagnosis anD treatment of myocarDitis: recommenDations of Ukrainian association of carDiology V. Kovalenko, E. Nesukay, S. Cherniuk, A. Kozliuk, R. Kirichenko SI «NCS «M.D. Strazhesko institute of cardiology» NAMS of Ukraine The recommendations are dedicated to contem- porary aspects of epidemiology, etiology, patho- genesis, diagnosis, etiology-based, pathogenetic and symptomatic treatment of myocarditis. Va rious pathogenetic mechanisms that cause the devel- opment and progression of inflammatory heart di- sease and cause dilatation and systolic dysfunction, lead to heart failure and the development of other complications of myocarditis are described in de- tail. These recommendations present the modern classification of myocarditis, approved in Ukraine, and modern algorithms for diagnosis and clinical management of patients, in particular the algo- rithm that justifies the appointment of glucocorti- coids for patients with myocarditis. The characte- ristics of different variants of myocarditis are also presented with clarifications concerning diagnosis and treatment. Much attention is paid to va rious ap- proaches to the etiotropic and pathogenetic treat- ment of myocarditis and their possible prospects. It is obvious that in order to standardi ze approach- es to the diagnosis and management of acute and chronic myocarditis, it is necessary to conduct large-scale multicenter studies and create spe- cial registries. In addition, in the current context of the COVID-19 pandemic, the pathological effects of SARS-CoV-2 as a trigger of myocarditis need fur- ther study, in particular in terms of impact on the prognosis and approaches to pathogenetic thera- py in such patients. Unification of terminology and approaches to diagnosis and clinical monito ring of patients with myocarditis can improve ma nagement tactics and increase the survival rate of such pa- tients. To identify high-risk patients (with arrhyth- mias, high probability of recurrence or transfor- mation of myocarditis into dilated cardiomyopa- thy) and candidates for heart transplantation, the most promising is the creation of special databas- es of such patients. key words: myocarditis, etiology, pathogenesis,
