Introduction:
Efferent medicine combines a group of hardware methods for
removing xenobiotics, auto-, exo- and endogenous toxins from the body, numbering
several dozen items; none of them is universal, since the substances to be eliminated
differ significantly in their physicochemical properties. The goal of efferent therapy
is to eliminate surgical endotoxemia in the provision of emergency surgical care to
patients with abdominal sepsis.
One of the main acute surgical pathologies of the abdominal cavity,
complicated by abdominal sepsis, is pancreonecrosis, the lethality of which reaches
30-40% or more.
The problem of surgical endotoxicosis that accompanies pancreatic necrosis
is very acute and has not been resolved to this day. Also, many organizational and
clinical problems of efferent medicine associated with
the creation of a general
scheme for the detoxification process in multidisciplinary medical institutions have
not been resolved.
Despite the fact that the problem is recognized by the majority of specialists
involved in the treatment of exo- and endotoxicoses, only infusion therapy with
forced diuresis is still used to correct it, which is associated not only with the high
cost of extracorporeal
detoxification methods, with the inaccessibility of these
methods, but also not knowing the full importance of endotoxin aggression in
aggravating the condition of a patient with surgical sepsis.
Evaluate the effectiveness of efferent therapy in destructive pancreatitis is a
promising direction in the study of this issue.
Purpose of the work: To improve the results of
treatment of patients with
pancreatic necrosis by including efferent therapy methods in complex intensive care.
